An applicant begins the EOI/medical underwriting process by submitting an EOI application, along with other information on medical history, to be utilized by Mutual of Omaha Insurance Company to make a decision about the coverage requested.

Evidence of Insurability Made Easy
When is Evidence of Insurability Required?
Generally, EOI is required for coverage in excess of any applicable guaranteed issue amount, when insurance is elected more than 31 days after you/Spouse is eligible, or if coverage is requested outside of your initial benefit enrollment period.
What is Guarantee Issue?
The amount of insurance applied for without answering any health questions or which does not require evidence of insurability.

Be sure to have the following information prior to starting the EOI application process.

- Your Date of Hire
- Your Current Salary
- Current Coverage Amounts (including Guaranteed Issue Amounts)
