Find a Claim form Online:
To access a claim form, go to MutualofOmaha.com/support/forms.
- On the forms page, select I am a Plan Member (Employee)
- Choose the relevant state.
- Then select the benefit you wish to file a claim for.

Accessing Claims Online Using the Employee Claims Portal
The Employee Portal gives employees the capability to view current claims, submit new short-term disability claims, report paid family and medical leave time, and download forms all from one location.
Getting Started
- Go to mybenefits.mutualofomaho.com/my-benefits/ui/registration
- Register for an account by filling out the necessary information. Click on Register.
- Users will be notified when they have completed the first step of created an account.
- An email will be sent with the final steps to finish setting up an account.
- Already have an account? Log in with your credentials.
